Sarcoma (soft tissue + bone) · Interventional immuno-oncology

Sarcoma is 100 diseases pretending to be one. We treat the one you have.

Sarcoma comes in many forms — our needle-based treatment is tuned to yours, for disease that has spread or stopped responding.

In plain terms

What we do, in three paragraphs.

Sarcoma is really more than 100 different cancers with very different biology, so a one-size-fits-all immune treatment doesn’t work — our approach is tuned to the specific subtype. We treat the tumor directly: ablation plus intratumoral immunotherapy, often using PEF so we can treat sarcomas near major vessels and nerves.
Some subtypes (UPS, dedifferentiated liposarcoma, ASPS, Kaposi) already respond to standard immunotherapy — for these we add intratumoral stimulants to extend and re-engage the response. Others (leiomyosarcoma, synovial sarcoma, GIST) are “cold”; breaking the tumor open releases its “fingerprints,” turns it “hot”, and the injected immunotherapy converts it — AblationVax, aiming for the abscopal effect at lung or liver spread.
For synovial sarcoma we can also tap cancer-testis antigens (shared markers like MAGE-A4 / NY-ESO-1) to broaden the immune response. We coordinate with your sarcoma team; most patients finish in about a week. Results vary.

One needle, one session: ablation plus immunotherapy delivered where the cancer lives.

AblationVax: the treated tumor teaches your immune system to attack cancer body-wide.

Real outcomes

Histology-driven re-engagement.

Pembrolizumab has activity in some subtypes but is largely inactive in leiomyosarcoma, synovial, and GIST. Our approach: re-engage immunity at the tumor site for cold-subtype disease.

Individual results vary. These outcomes represent specific patient cases and are not guaranteed for all patients.

Where our approach fits

Treatment options by stage.

Find your diagnosis below to see what standard care offers, where we add to it, and what role our approach plays.

Localized

Standard care
Surgery ± radiation; neoadjuvant chemo for high-grade.
What we add
Our approach is rarely first-line.
Standard care first

Oligometastatic

Standard care
Limited number of sites.
What we add
Local control + intratumoral immune amplification.
Local + systemic strategy

Metastatic, immune-responsive subtypes

Standard care
UPS, DDLPS, ASPS, Kaposi — pembrolizumab has activity.
What we add
Our approach extends and re-engages the response.
Adjunct + re-engagement

Our primary focus

Metastatic, immune-cold subtypes

Standard care
Leiomyosarcoma, synovial, GIST — cold to checkpoint monotherapy.
What we add
Convert cold to hot at the tumor site.

Each subtype has distinct immune biology — our intratumoral therapies are tuned to each.

Cold-tumor activation

Beyond ablation alone

Destroy the tumor, then teach your immune system to finish the job.

Standard approaches
Our approach — AblationVax
Doxorubicin-based combos → subtype-specific agents → TCR-T for selected synovial.
Ablation + intratumoral immunotherapy for cold-subtype re-engagement and post-targeted progression.

IV pembrolizumab: activity only in selected histologies.
Delivered directly into the tumor rather than through the bloodstream. The aim is to concentrate treatment at the tumor site and limit whole-body exposure.

Limited options after standard sequences in cold subtypes.
Histology-aware intratumoral therapies tuned to each subtype’s immune biology.

Why this approach for sarcoma?

Treats tumors near critical structures

Retroperitoneum, extremity, chest wall, viscera — non-thermal PEF allows treatment near major vessels and nerves.

Converts “cold” subtypes into “hot” targets

Leiomyosarcoma, synovial, and GIST are cold to checkpoint monotherapy. Ablation + intratumoral immunotherapy addresses the root cause.

Aims for systemic response

The investigational goal is to determine whether treating one tumor can stimulate immune activity at untreated sites in the lung, liver, and elsewhere.

Sarcoma-specific breakthrough

Histology-aware intratumoral cocktails.

“Sarcoma is 100 diseases pretending to be one. The right intratumoral combination for a leiomyosarcoma isn’t the same as for a synovial sarcoma. We tune the immune amplification to the histology.”

— Dr. Jason R. Williams

Extending pembrolizumab activity

For UPS / DDLPS / ASPS / Kaposi, intratumoral OX40 / CD40 + CpG is a rational priming strategy to extend and re-engage checkpoint response.

Synovial sarcoma antigens

Cancer-testis antigens (MAGE-A4 / NY-ESO-1) are conserved across synovial sarcoma — ablation antigen release can engage broader responses beyond TCR therapy.

Leiomyosarcoma cold conversion

LMS is one of the most immune-cold subtypes — ablation + intratumoral immunotherapy addresses the cold microenvironment directly.

“Sarcoma histology drives immunotherapy choice. We pick the intratumoral therapies based on that subtype’s immune biology — not one-size-fits-all.” — Dr. Williams

Beyond the procedure: whole-patient support.

We optimize your entire system for the best possible response — and coordinate with the team you already have.

Gut microbiome optimization

Diet, probiotics, and related therapies to support a stronger immunotherapy response.

Off-label protocols

Strategic use of repurposed medications to enhance immune response.

Personalized cocktails

Multi-agent immunotherapy tailored to your specific cancer.

Oncologist coordination

We work alongside your current chemo and radiation team.

What to expect

Your path to treatment.

All outpatient

Is this right for you?

You may be a candidate if:

✓Metastatic soft-tissue or bone sarcoma progressing on standard sequences
✓Immune-cold subtype (LMS, synovial, GIST) seeking immune activation
✓UPS / DDLPS / ASPS / Kaposi after pembrolizumab progression
✓Synovial sarcoma after MAGE-A4 TCR therapy progression
✓Oligoprogressive disease on systemic therapy

It may not be suitable if:

—Diffuse miliary metastatic pattern without a focal target
—Pathologic fracture risk without an orthopedic stabilization plan
—Active severe infection
—Uncorrectable bleeding disorders
—Very poor performance status (ECOG ≥ 3)

Each case is individually evaluated by Dr. Williams’ team.
