Prostate cancer · Interventional immuno-oncology

Prostate cancer taught us that intratumoral delivery isn’t optional. It’s the only way in.

For advanced prostate cancer that has outlasted hormone therapy and other treatments — we treat the tumor directly and reawaken the immune response.

In plain terms

What we do, in three paragraphs.

Prostate cancer is one of the most immune-“cold” cancers there is — standard immunotherapy that works elsewhere has consistently failed here, because the area around the tumor (especially in bone) actively suppresses the immune system. Our approach breaks that pattern by treating the tumor directly: ablation (often cryoablation, which has a long history in the prostate) plus intratumoral immunotherapy.
Breaking the tumor open releases its hidden “fingerprints” and turns it “hot”, and injecting immune drugs into that spot — checkpoint inhibitors plus stimulants like CpG and OX40/CD40 agonists — aims to wake up an immune attack and trigger the abscopal effect at untreated bone or other sites.
It’s designed for metastatic castration-resistant prostate cancer (mCRPC) progressing after hormone therapies, chemotherapy, PARP inhibitors, or Pluvicto, and for oligometastatic disease. We coordinate with your oncology and urology teams; most patients finish standard treatment in about a week as outpatients. Results vary.

One needle, one session: ablation plus immunotherapy delivered where the cancer lives.

AblationVax: the treated tumor teaches your immune system to attack cancer body-wide.

Real outcomes

After Pluvicto, after PARP, after everything.

mCRPC patients today have multiple lines — AR inhibitors, taxanes, PARP inhibitors, Pluvicto, Provenge. After progression on these, options narrow. Prostate cancer is the textbook immune-cold tumor; our approach is designed to break that cold-tumor pattern.

Individual results vary. These outcomes represent specific patient cases and are not guaranteed for all patients.

Where our approach fits

Treatment options by stage.

Find your diagnosis below to see what standard care offers, where we add to it, and what role our approach plays.

Localized high-risk

Standard care
Radical prostatectomy or radiation + ADT.
What we add
Our approach is rarely first-line.
Standard care first

Oligometastatic

Standard care
Limited bone or visceral sites.
What we add
Local control of all sites + intratumoral immune amplification.
Local + systemic strategy

Our primary focus

mCRPC (castration-resistant)

Standard care
AR inhibitor → taxane → PARP (if HRR+) → Pluvicto.
What we add
Re-engagement during or after these sequences.

The bone microenvironment, where most metastases live, is profoundly immunosuppressive — we convert the cold tumor to hot at the site itself.

Systemic immune activation

Post-Pluvicto / post-PARP

Standard care
Limited remaining options.
What we add
Intratumoral immune cocktails for re-engagement.
Re-engagement

Beyond ablation alone

Destroy the tumor, then teach your immune system to finish the job.

Standard approaches
Our approach — AblationVax
AR axis → taxane → PARP → Pluvicto → Provenge (selected).
Ablation + intratumoral immunotherapy at any progression step, designed for immune-cold disease.

IV checkpoint blockade: largely ineffective in unselected prostate cancer.
Delivered directly into the tumor rather than through the bloodstream. The aim is to concentrate treatment at the tumor site and limit whole-body exposure.

Local cryo for selected localized disease.
Cryoablation + intratumoral immunotherapy for systemic immune activation.

Why this approach for prostate cancer?

Treats tumors near critical structures

Pelvic anatomy, neurovascular bundles, rectum, urethra. Cryoablation has a long prostate history; PEF enables treatment where heat is risky.

Converts “cold” tumors into “hot” targets

Prostate’s immune-cold biology is well-established. Ablation + intratumoral immunotherapy addresses the root cause: local immunosuppression.

Aims for systemic response

The investigational goal is to determine whether treating one tumor can stimulate immune activity at untreated bone or visceral metastases.

Prostate-specific breakthrough

Breaking the immune-cold phenotype.

“Prostate cancer has been the disease where conventional immunotherapy has disappointed most. The tumor microenvironment is engineered to suppress immune activity — our approach changes the microenvironment at the source.”

— Dr. Jason R. Williams

TLR agonists + costimulation

CpG and OX40 / CD40 agonists delivered intratumorally generate the inflammatory danger signal and costimulatory drive prostate tumors lack.

Bone-targeting strategies

For bone-dominant disease, integrating PSMA-targeted approaches with intratumoral immunotherapy is an active research area.

The combination

CpG, OX40 / CD40, and checkpoint inhibitors delivered together intratumorally — immune responses in a tumor type historically refractory to immunotherapy.

“Prostate cancer is the disease that taught me intratumoral delivery wasn’t optional — it was the only way to overcome the local immunosuppression.” — Dr. Williams

Beyond the procedure: whole-patient support.

We optimize your entire system for the best possible response — and coordinate with the team you already have.

Gut microbiome optimization

Off-label protocols

Strategic use of repurposed medications to enhance immune response.

Personalized cocktails

Multi-agent immunotherapy tailored to your specific cancer.

Oncologist coordination

We work alongside your current chemo and radiation team.

What to expect

Procedure ~1 hour

Recovery ~2 hours

All outpatient

Is this right for you?

You may be a candidate if:

✓mCRPC progressing on AR inhibitors and taxane
✓Progression on PARP inhibitors (BRCA / HRR+)
✓Progression on Pluvicto (Lu-177 PSMA-617)
✓Oligometastatic disease with limited progressing sites
✓Seeking immune options in a disease refractory to checkpoint blockade

It may not be suitable if:

—Diffuse miliary metastatic pattern without a focal target
—Severe genitourinary obstruction without a resolution plan
—Active severe infection
—Uncorrectable bleeding disorders
—Very poor performance status (ECOG ≥ 3)

Each case is individually evaluated by Dr. Williams’ team.
